CAGRILINTIDE · THE ESSENTIALS

You have eaten.
Your body says: enough.

Cagrilintide aims to strengthen a natural fullness signal. The practical idea: finding it easier to finish a meal with less food.

AMYLIN · A DIFFERENT SIGNAL

What exactly is it?

It is a long-acting amylin analogue: a peptide developed from a fullness hormone. Molecular modifications extend its presence in the body. It is also known in research as AM833. ↗

It is being investigated in trials and is not FDA approved. The findings below explain its potential; they are not instructions for use. ↗

Cagrilintide

One molecule. An amylin analogue. It activates amylin and calcitonin receptors; it is not a GLP-1 agonist. ↗

CagriSema

Two molecules: cagrilintide + semaglutide. The combination’s outcome needs to be read separately from cagrilintide alone. ↗

HOW IT WORKS

Finding it easier to stop.
Less bargaining at the table.

Fullness is the feeling that you have eaten enough. It is different from nausea or losing all interest in food.

The meal ends

Less food may feel sufficient

An explanatory schematic of the natural signal. AMY receptors combine the calcitonin receptor with proteins called RAMPs. Molecular studies explain binding; they do not measure an individual’s future weight loss. ↗

What may become easier

Eating a smaller portion and stopping without feeling that something is still missing. That is the practical meaning of a fullness signal. The effect is not identical for everyone.

“Food noise” is an experience, not a percentage.

Some people describe fewer persistent thoughts about food. Others feel full sooner while cravings remain. A weight chart does not tell us how many experienced each change.

The benefit needs to fit your life.

No appetite at all, fatigue that stops daily activities or persistent nausea are not goals. How you eat, how you feel and what you can do each day matter too.

REDEFINE 1 · 68 WEEKS

Four groups.
The same question.

How did weight change? 3,417 adults without type 2 diabetes, with obesity or overweight and a related condition. All groups received lifestyle support.

Average weight loss

Including people who stopped treatment

The main analysis estimates the effect regardless of whether participants continued treatment. This is the initially displayed perspective.

Cagrilintide · n=302−11.5%
Semaglutide · n=302−14.9%
CagriSema · n=2,108−20.4%
Placebo · n=705−3.0%

If everyone had followed treatment

A statistical estimate for a scenario in which everyone continued treatment. It is not a promised result for someone who follows it perfectly.

Cagrilintide · n=302−11.8%
Semaglutide · n=302−16.1%
CagriSema · n=2,108−22.7%
Placebo · n=705−2.3%

Trial target doses: 2.4 mg for each component, weekly. Values are averages at week 68, not a weight-loss trajectory or dosing recommendation. ↗

How to read the difference

11.5% and 20.4% answer different questions: the first is for cagrilintide alone, the second for CagriSema. We do not add the component results to predict the combination. ↗

What came before phase 3? The 26-week trial

In phase 2, 706 adults without diabetes were assigned across several groups. At 26 weeks, the analysis assuming adherence showed: ↗

Cagrilintide · 4.5 mg/week−10.8%
Liraglutide · 3 mg/day−9.0%
Placebo−3.0%

Different dose, duration and trial. The liraglutide comparison was direct, but these values should not be ranked against the 68-week trial. ↗

CAGRILINTIDE + SEMAGLUTIDE

Two signals.
One result measured together.

Amylin and GLP-1 offer two routes to influence appetite. CagriSema brings them into the same research strategy.

What a comparison within one trial tells us

REDEFINE 1 lets you see the components and combination over the same duration in the same population. That is more useful than comparing two percentages picked from different trials. ↗

Against tirzepatide? The trial matters.

In the REDEFINE 4 topline announcement at 84 weeks, CagriSema did not meet the non-inferiority criterion against tirzepatide. In plain terms, the trial did not establish at least comparable weight loss within its predefined margin. This concerns the combination, not cagrilintide alone. ↗

WEIGHT AND DIABET

What about blood sugar?
It is a different measure.

HbA1c reflects average blood sugar over roughly three months. Its reduction here is expressed in percentage points.

Phase 2 · 92 adults with type 2 diabetes · 32 weeks
GroupHbA1cWeight
Cagrilintide · n=30−0.9 pp−8.1%
Semaglutide · n=31−1.8 pp−5.1%
CagriSema · n=31−2.2 pp−15.6%

Both components: target dose of 2.4 mg/week. The HbA1c difference between the combination and semaglutide did not reach statistical significance in this small trial (p=0.075). A larger number on screen alone does not establish superiority. ↗

WHAT WE KNOW. WHAT STILL NEEDS ANSWERS.

Weight matters.
So does life afterwards.

Results need to be considered alongside tolerability, energy and questions that still lack a clear answer.

01

The heart

REDEFINE 3 examines whether CagriSema changes cardiovascular event risk. Lower weight or blood pressure cannot replace evidence about heart attacks or strokes. ↗

02

What happens after stopping

We do not use semaglutide withdrawal findings to calculate weight regain after cagrilintide. For the molecule alone, the evidence presented here does not sufficiently answer that long-term question. ↗

03

Muscles, energy, everyday life

A weight-loss percentage measures total body weight. It does not establish muscle preservation or better energy. Those are separate outcomes that need separate measurements. ↗

EXPERIENCES PEOPLE SHARE

Beyond the averages,
very different days.

Three public accounts, paraphrased. All also involve other substances; products and diagnoses were not independently verified. They do not show how often an effect occurs.

01 / Reddit

Progress again after a plateau.

After several treatment changes, this person reported losing about 2.7 kg in a month and feeling better. They were also using tirzepatide. They described only mild headaches and said the fatigue they had feared had not appeared. A mostly positive experience, but the combination prevents attributing the outcome to cagrilintide alone.

Original account ↗
02 / Reddit

Disappointment first. Then an update.

Expectations were high after reading other people’s experiences. The first impression was intense fatigue without the hoped-for appetite change. They were already using tirzepatide. Later in the same discussion, they reported a lower appetite and returning energy. This shows why a post captures a moment; it does not establish a faulty batch or a rule for when an effect begins.

Original account ↗
03 / Reddit

Appetite disappeared. So did energy for an ordinary day.

After adding cagrilintide while using retatrutide, this person described nausea, dizziness, anxiety and difficulty eating even something simple. They cancelled an exercise activity because of exhaustion and said they did not want to repeat the experience. The combination and unverified product prevent establishing the cause. At that moment, the lack of hunger did not compensate for how they felt.

Original account ↗

TOLERABILITY · CAGRILINTIDE ALONE

Less hunger
should not mean feeling worse.

Digestive symptoms stand out among adverse effects in trials. The table below concerns cagrilintide, not CagriSema.

Sponsor data; cagrilintide 2.4 mg/week. A participant may report more than one reaction, so rows must not be added. Other publications may use different observation windows; their percentages have not been mixed here. ↗

Fatigue in personal accounts

Some people describe exhaustion or low energy. These accounts do not establish a frequency or prove causation. Symptoms that interfere with daily life deserve assessment, not interpretation as a sign that it is “working”. ↗

Pregnancy, breastfeeding and online products

There is no FDA-approved cagrilintide label establishing safe use in these circumstances. A product sold online for “research” is not equivalent to the controlled product in a clinical trial. ↗

When to seek help

Repeated vomiting, inability to keep fluids down, severe persistent abdominal pain or fainting need prompt assessment. For difficulty breathing or swelling of the tongue or throat, call 112 in Romania/the EU or your local emergency number. These are general warning signs, not a list of confirmed molecule-specific reactions.

06 / FAQ

Your questions.
Clear answers.

The dossier, explained simply. Open a question for the answer and its source.

What is cagrilintide?

Cagrilintide is a long-acting amylin analogue studied for appetite control and weight loss. Amylin is a hormone involved in fullness. The molecule remains investigational.

Source & context ↗

Is cagrilintide a GLP-1 medicine?

No. Cagrilintide acts at amylin and calcitonin receptors. Semaglutide is a GLP-1 agonist. The two are studied together in the combination called CagriSema.

Source & context ↗

Are cagrilintide and CagriSema the same?

No. CagriSema combines cagrilintide with semaglutide. A combination result cannot be presented as a result for cagrilintide alone.

Source & context ↗

How much weight was lost with cagrilintide in trials?

In REDEFINE 1, cagrilintide alone produced average weight loss of 11.5% at 68 weeks, versus 3.0% with placebo, in the analysis including discontinuation. Participants did not have type 2 diabetes. A group average does not predict a personal result.

Source & context ↗

Why are both 11.5% and 11.8% reported?

They are two analyses of the same trial. 11.5% estimates the effect regardless of continued treatment; 11.8% assumes participants had followed it. They are not two doses or two promised outcomes.

Source & context ↗

Is cagrilintide approved?

At the 15 October 2026 review, cagrilintide remains investigational. The FDA states that it is not a component of an FDA-approved drug. An online listing does not establish authorisation or product quality.

Source & context ↗

Is it studied daily or weekly?

The trials shown used weekly administration. Chart doses describe the research, not an approved schedule for home use.

Source & context ↗

Can it reduce constant thoughts about food?

Some accounts describe this; others report different changes in fullness and cravings. Accounts do not establish a probability, and trial weight loss does not automatically measure “food noise”.

Source & context ↗

Which side effects have been observed?

Trials have particularly reported nausea, constipation, diarrhoea and vomiting. Fatigue also appears in accounts, but selected posts cannot establish its frequency or cause. The dedicated section keeps these types of information separate.

Source & context ↗Explore the Side effects section ↗

Does it also help blood sugar?

There is type 2 diabetes research on the molecule alone and on CagriSema. This does not make cagrilintide an approved diabetes treatment or justify replacing insulin or another prescribed medicine.

Source & context ↗

Has heart protection been established?

The weight-loss results presented do not establish fewer heart attacks or strokes with cagrilintide alone. REDEFINE 3 examines cardiovascular events with the CagriSema combination.

Source & context ↗

What happens after stopping?

The evidence presented is insufficient to estimate long-term weight regain after cagrilintide alone. We do not automatically transfer regain percentages from semaglutide or other molecules.
