WHAT IS TIRZEPATIDE

Two signals.
One molecule.

Tirzepatide is the active ingredient in Mounjaro and Zepbound. It activates GIP and GLP-1 receptors, two pathways involved in blood sugar and appetite control.

39 AMINO ACIDS

Chain schematic, not an atomic model. Highlights show modified positions 2 and 13 and the side-chain attachment at position 20. ↗

Why does it not disappear immediately?

A side chain containing 20 carbon atoms helps it bind to albumin, a blood protein. This makes the molecule last longer. GIP and GLP-1 are two targets of the same peptide; this is not a mixture of two medicines. ↗

The name on the box matters.

01

Mounjaro · EU

In the EU, it is authorised for type 2 diabetes from age 10 and for weight management in eligible adults, alongside diet and physical activity. ↗

02

Zepbound · US

In the US, it is authorised for weight management and moderate or severe obstructive sleep apnoea in adults with obesity. Indications differ between countries. ↗

03

Who is eligible for weight treatment?

The EU indication covers adults with BMI at least 30, or at least 27 plus a weight-related condition. A clinical assessment determines suitability. ↗

Checked: 14 October 2026. EU authorisation does not confirm pharmacy availability or reimbursement in Romania.

IN EVERYDAY LIFE

Easier to stop.
Less negotiating with hunger.

For some people, the important change is that food takes up less mental space. “Food noise” describes that preoccupation; it is not a diagnosis.

Hunger may become easier to manage.

A six-week trial involving 114 adults without diabetes found less hunger, fewer cravings and lower intake at a test meal with tirzepatide. This is a measured observation, not only a community impression. ↗

A smaller portion can feel sufficient.

Stomach emptying slows, but this effect is strongest initially and diminishes later. Signals to the brain also regulate appetite; food remaining in the stomach is not the whole explanation. ↗

One day does not tell the whole story.

Weight outcomes were assessed over many months. A larger meal or a hungry day cannot be compared with the average of a 72-week trial. The overall trend and tolerability matter. ↗

WEIGHT · HUMAN RESULTS

What the scale showed.
And what a direct comparison tells us.

Trial averages describe a group. They do not predict one person’s result. The values below account for treatment discontinuation.

SURMOUNT-1

Against placebo

2,539 adults without diabetes, with obesity or overweight plus a related condition. All groups received diet and activity support.

Mean weight change at 72 weeks

Tirzepatide · 5 mg−15.0%
Tirzepatide · 10 mg−19.5%
Tirzepatide · 15 mg−20.9%
Placebo−3.1%

Doses identify research groups; they are not treatment recommendations. ↗

SURMOUNT-5

Against semaglutide

751 adults with obesity, without diabetes; 72 weeks. An open-label trial: participants knew which treatment they received.

Mean weight change

Tirzepatide · 10 / 15 mg−20.2%
Semaglutide · 1.7 / 2.4 mg−13.7%

Maximum tolerated weekly injectable doses. Mean difference: 6.5 percentage points. This comparison does not cover oral semaglutide or the 7.2 mg dose. ↗

6.5 points

20.2 minus 13.7. That is the mean weight difference in the comparison above. Treatment choice also depends on other conditions, tolerability and the ability to continue.

TYPE 2 DIABETES

More control.
Over blood sugar too.

HbA1c is a blood test reflecting blood sugar over roughly the past three months. In diabetes, lowering it is a separate goal from weight loss.

SURPASS-2 · 40 WEEKS

A comparison conducted in diabetes.

1,879 adults with type 2 diabetes. HbA1c fell by 2.01–2.30 percentage points with tirzepatide 5–15 mg, compared with 1.86 with semaglutide 1 mg. ↗

The same names, a different question.

SURPASS-2 compared diabetes control. SURMOUNT-5 compared weight loss in people without diabetes, using different semaglutide doses. Keep the two results separate.

BEYOND WEIGHT

Sleep and heart health.
Results for specific groups of people.

A benefit observed in one condition does not automatically apply to everyone. Here is who was studied and what changed.

SURMOUNT-OSA

Fewer breathing interruptions.

Adults with obesity and moderate or severe obstructive sleep apnoea, followed for 52 weeks. In the trial without PAP treatment, AHI fell by 25.3 events/hour with tirzepatide versus 5.3 with placebo. ↗

AHI counts breathing pauses and reductions per hour of sleep. It is not a percentage. Decisions about CPAP require clinical reassessment.

SUMMIT

Less worsening of heart failure.

731 people with obesity and heart failure with preserved ejection fraction. Over a median follow-up of 104 weeks, cardiovascular death or worsening heart failure occurred in 9.9% with tirzepatide versus 15.3% with placebo. ↗

The difference was mainly driven by fewer worsening events. The trial did not separately establish fewer cardiovascular deaths.

SURPASS-CVOT

What about heart attack or stroke?

In people with type 2 diabetes and cardiovascular disease, tirzepatide was compared with dulaglutide, not placebo. Cardiovascular death, heart attack or stroke: 12.2% versus 13.1%. It established noninferiority, but not statistical superiority. In other words, this trial does not establish that tirzepatide was better for this outcome. ↗

In the EU, sleep apnoea data are included in the product information within weight management; they are not a separate sleep apnoea indication. ↗

LIVER · PHASE 2 RESEARCH

Less inflammation.
A research direction worth following.

MASH means metabolic fatty liver with inflammation and injury. Fibrosis is scarring caused by repeated damage.

SYNERGY-NASH · 52 WEEKS

What did the biopsy show?

190 participants with MASH and F2–F3 fibrosis. MASH resolution without worsening fibrosis: 44%, 56% and 62% with tirzepatide 5, 10 and 15 mg, versus 10% with placebo. ↗

Promising does not mean every question is answered.

This was a phase 2 trial and does not establish prevention of cirrhosis or longer survival. The result is not an approved indication for every form of fatty liver.

WHAT COMES AFTER WEIGHT LOSS

The result matters.
So does keeping it.

Two useful questions: what happens after stopping, and how much of the lost weight is fat?

SURMOUNT-4

After stopping, weight can return.

After 36 weeks of tirzepatide, 670 participants continued treatment or switched to placebo. Over the next 52 weeks, weight changed by −5.5% with continuation and +14.0% after switching to placebo. ↗

These percentages use weight at week 36 as their baseline. Randomisation included people who reached that stage; the result does not describe everyone starting treatment.

BODY COMPOSITION

Lean mass deserves attention.

The SURMOUNT-1 DXA substudy included 160 people. At 72 weeks, pooled tirzepatide groups showed fat mass −33.9% and lean mass −10.9%. ↗

The percentages use different starting amounts and cannot be added. Lean mass is not synonymous with muscle. This analysis alone cannot tell how much strength someone lost.

PERSONAL ACCOUNTS

The scale does not tell
the whole story.

Three public accounts read on Drugs.com and paraphrased. These are selected experiences, not a survey or evidence of an effect’s frequency or cause.

01 / Drugs.com

Fewer trips to the kitchen.

One person describes four months with less stress- or boredom-driven snacking. Meals ended at fullness; the early tiredness had passed.

02 / Drugs.com

Results matter. So does tolerability.

After months without problems, a dose increase was followed by nausea, diarrhoea and fatigue. They valued the weight loss but preferred slower progress.

03 / Drugs.com

At goal, facing a difficult decision.

One person had reached their desired weight. Nausea and diarrhoea continued; stopping was under consideration, alongside fear of regaining weight.

SIDE EFFECTS

What can be difficult.
And what deserves attention.

Digestive reactions are the most frequent. More discomfort does not mean more benefit.

Nausea25%29%28%8%
Diarrhoea19%21%23%8%

Discuss beforehand

Discuss pancreatitis, gallbladder problems, severely delayed stomach emptying, retinopathy and diabetes medicines. Insulin and sulfonylureas can increase hypoglycaemia risk. Inform your medical team before anaesthesia. ↗

The US label contraindicates use with a personal or family history of medullary thyroid cancer or MEN2. The warning stems from rodent tumours; human relevance is not established. ↗

Pregnancy and contraception

Do not use it during pregnancy. Plan discontinuation with your clinician before a planned pregnancy. ↗

The contraceptive pill may be less effective. Guidance recommends a non-oral method or adding a barrier method for four weeks after starting and after every dose increase. ↗

When to seek help

Severe persistent abdominal pain, repeated vomiting or inability to keep fluids down need prompt assessment. Difficulty breathing or swelling of the tongue or throat is an emergency: call 112 in Romania/the EU or your local emergency number. ↗

06 / FAQ

Your questions.
Clear answers.

The dossier, explained simply. Open a question for the answer and its source.

What is tirzepatide and what does it help with?

Tirzepatide activates GIP and GLP-1 receptors. It can reduce hunger and helps control blood sugar. It is the active ingredient in Mounjaro and Zepbound; indications depend on country and product.

Source & context ↗

Tirzepatide or semaglutide: which produced more weight loss in the trial?

In SURMOUNT-5, the 72-week average was −20.2% with tirzepatide and −13.7% with semaglutide. Participants were adults with obesity without diabetes, using the weekly injectable doses specified in this dossier. The result does not automatically determine an individual choice.

Source & context ↗

How quickly do you lose weight with tirzepatide?

There is no guaranteed timeline or number of kilograms. SURMOUNT-1 assessed outcomes at 72 weeks. Appetite changes and weight changes do not necessarily follow the same pace.

Source & context ↗

Is it only for people with diabetes?

No. The European weight-management indication includes eligible adults without diabetes. Mounjaro also has a separate type 2 diabetes indication.

Source & context ↗

Does it help with sleep apnoea?

Trials in adults with obesity and moderate or severe obstructive sleep apnoea found fewer breathing interruptions during sleep. This does not justify stopping CPAP without reassessment.

Source & context ↗

What happens if you stop tirzepatide?

In SURMOUNT-4, switching to placebo after 36 weeks of treatment was followed by weight regain on average. Discuss maintaining the result before stopping.

Source & context ↗

Is muscle lost too?

Lean mass can decrease; it includes muscle, water and other tissues. DXA does not directly measure strength. Weight loss is not exclusively fat loss.

Source & context ↗

What are the common side effects?

Nausea, diarrhoea, vomiting and constipation are common. The dedicated section shows trial percentages and symptoms that need medical help.

Source & context ↗Explore the Side effects section ↗

Can it affect the contraceptive pill?

Yes. Guidance recommends a non-oral method or an additional barrier method for four weeks after starting and after every dose increase. Discuss the choice with your clinician.

Source & context ↗

Can it be combined with semaglutide?

Combining Zepbound with a GLP-1 agonist such as semaglutide is not recommended in the US label. More products do not automatically mean more benefit.
