TESAMORELIN · THE ESSENTIALS

Fat around the organs.
A change from within.

Tesamorelin is a peptide that stimulates release of the body’s own growth hormone. Its appeal comes from a distinct clinical effect: it can reduce visceral fat in adults with HIV and lipodystrophy even when the scale changes very little.

44 amino acids

A synthetic GHRH analogue, with a modification at the chain’s end that improves stability. The diagram’s 44 dots represent amino acids; this is not a 3D molecular structure. ↗

Visceral and subcutaneous fat

The first lies deep around organs; the second lies under the skin. Waist size and appearance offer clues, but do not separately quantify the visceral depot. Trials used CT imaging. ↗

A specific US indication

Egrifta is authorised in the US for excess abdominal fat in adults with HIV and lipodystrophy. It has no general approval for weight management, fitness or anti-ageing. ↗

MECHANISM · EXPLAINED SIMPLY

From the pituitary
to fat metabolism.

Tesamorelin uses the GHRH signal. GH and IGF-1 are downstream steps, while the abdomen is where researchers measure a response.

EXPLANATORY SCHEMATIC · NOT A CLINICAL SIMULATION

The pituitary receives the message.

Tesamorelin binds to the pituitary’s GHRH receptor. This receptor is the entry point into the hormonal axis. The gland must function: certain pituitary and hypothalamic conditions are contraindications in the US label. ↗

The body releases GH and IGF-1 rises.

GH acts on several tissues and stimulates IGF-1 production, particularly in the liver. Higher IGF-1 confirms a biological response, but more is not automatically better. Persistent elevations require medical monitoring. ↗

The visceral depot can decrease.

In trials, stimulation of this hormonal axis was associated with less visceral fat. This was measured in defined groups, not a guarantee that every fat depot will shrink. The schematic explains the pathway without simulating a personal outcome. ↗

TWO PIVOTAL TRIALS

What human trials
measured.

Two phase 3 trials compared tesamorelin with placebo in adults with HIV and lipodystrophy. The primary outcome was CT-measured visceral fat.

Falutz · 2007

412 randomised · HIV/lipodystrophy · 26 weeks

Phase 3, double-blind, versus placebo. The chart uses the FDA analysis: 273 in the active arm and 137 on placebo. The NEJM paper uses a different analysis and reports −15.2% vs +5.0%; these come from the same cohort. The study intervention was 2 mg subcutaneous tesamorelin in the historical formulation; this identifies the trial arm, not a use regimen. ↗

Falutz · 2010

404 randomised · HIV/lipodystrophy · 26 weeks

The second pivotal trial, with 270 and 126 in the FDA efficacy table. The paper reports −10.9% vs −0.6%. For consistency, both charts use only the FDA model. The trials formed part of the Theratechnologies programme. The study intervention was 2 mg subcutaneous tesamorelin in the historical formulation; this identifies the trial arm, not a use regimen. ↗

BODY COMPOSITION

Why the scale
can stay almost unchanged.

Fat in one region, lean mass and total weight are different measurements. Here they are shown together from the same two trials.

Waist and kilograms

At 26 weeks, active arms had mean weight changes of −0.4 kg and +0.5 kg despite less visceral fat. Mean waist reductions were 3 cm and 2 cm, versus 1 cm in each placebo arm. These are group means, not a personal target. ↗

What lean mass includes

Reported mean increases were 1.3 kg and 1.2 kg in the active arms. This measurement includes muscle, other tissues and water. Fluid retention can affect it, and the number alone does not demonstrate muscle hypertrophy or greater strength. ↗

LIVER · HUMAN TRIALS

A promising result
for liver fat.

Research also examined liver fat in people with HIV. This is a different question from weight loss.

Stanley · 2014 · 6 months

50 people randomised, 28 to tesamorelin and 22 to placebo. Median change in the hepatic lipid-to-water ratio was −2.0 vs +0.9 percentage points. Adjusted difference: −2.9 pp, p = 0.003. This was a small trial using an imaging endpoint. ↗

Stanley · 2019 · 12 months

Phase 2, double-blind trial with 61 enrolled: 31 randomised to tesamorelin and 30 to placebo; 30 in each arm received the reported intervention. The adjusted difference in hepatic fat fraction was −4.1 percentage points versus placebo, 95% CI −7.6 to −0.7; p = 0.018. The relative effect was −37%: that has a different denominator and does not mean 37 percentage points. ↗

What this means for fibrosis

Less fibrosis progression was observed in a subgroup with paired biopsies. This is exploratory evidence in a small sample. It does not establish reversal of existing liver scarring or efficacy for everyone with fatty liver without HIV. The trial had NIH/NIAID funding. ↗

OTHER CLINICAL QUESTIONS

What has been studied
beyond the approved indication.

Research also exists without HIV, but in selected groups. Cognition and glucose safety findings need careful interpretation.

Obesity without HIV · 2012

Makimura studied 60 adults with abdominal obesity and reduced GH secretion for 12 months versus placebo. Visceral fat: −16 vs +19 cm²; between-group effect −35 cm², 95% CI −58 to −12, p = 0.003. The subcutaneous-fat difference was not significant. This population does not represent all overweight adults. ↗

Cardiovascular markers

In that trial, triglycerides changed by −26 vs +12 mg/dL and carotid wall thickness by −0.03 vs +0.01 mm. These are intermediate outcomes, not prevented heart attacks. The US label states that long-term cardiovascular safety is not established. ↗

Type 2 diabetes · 2017

Clemmons compared two exposure levels with placebo in 53 adults with stable diabetes for 12 weeks. No significant overall difference in insulin response or glycaemic control was found. Short follow-up and a small sample do not cancel the risk signal in the 26-week trials. Funding: Theratechnologies. ↗

Cognition · 2025

Ellis enrolled 73 people with HIV and abdominal obesity. At six months, comparison with usual care showed no significant main cognitive difference: p = 0.673. There was no placebo arm and statistical power was limited. This does not confirm a memory benefit or an Alzheimer’s treatment. ↗

The same cohorts, additional analyses

Falutz 2008 follows the first trial’s extension; the pooled Falutz 2010 paper combines the two pivotal trials. Russo 2024 examines an integrase-inhibitor subgroup of the 2019 liver trial. Lake 2021 examines fat density in selected pivotal-trial responders. These add perspectives rather than four independent clinical confirmations. ↗ ↗ ↗ ↗

PRODUCTS & AUTHORISATION

The same molecule name.
Different formulations.

Product, indication and region matter. A vial sold as a “research peptide” is not equivalent to the medicine evaluated in trials.

EGRIFTA WR and SV

Both are US pharmaceutical formulations, but presentation, dose and storage differ. FDA states that they are not substitutable. The pivotal trials used historical Egrifta; the WR label draws on those trials and pharmacokinetic comparisons between formulations. ↗

Europe and Romania

The European Egrifta application was withdrawn in 2012. EMA describes concerns about clinical relevance, IGF-1 and missing long-term safety data. Withdrawal should not simply be called a “ban”. Romanian availability, import and reimbursement are not confirmed here. ↗

Tesamorelin and GLP-1 medicines

Tesamorelin stimulates the GHRH–GH–IGF-1 axis. Semaglutide and tirzepatide use other hormonal targets and have different indications. Different trial populations cannot support a direct ranking of fat reduction or safety. Online accounts do not validate the combinations they describe. ↗

PERSONAL EXPERIENCES · REDDIT

What people say
after trying it.

Five different experiences paraphrased from inspected posts and comments. Products, diagnoses and causes of change are not medically confirmed. Selected accounts do not show how common these outcomes are.

01 / Reddit · Positive

A flatter-looking abdomen

After about six weeks, one person says the abdomen looks flatter. In a reply, they clarify they were also in a calorie deficit and describe early fluid retention. Change was not quantified by CT or MRI and cannot be attributed solely to the peptide.

02 / Reddit · Mixed

Changed contour, stable weight

After eight weeks, one person describes a tighter midsection and better workouts, but stable weight. Comments also mention retatrutide, DSIP and more regular sleep. These concurrent changes prevent separating tesamorelin’s contribution.

03 / Reddit · Sleep

Easier sleep, harder mornings

After two weeks, one person reports falling asleep more easily and vivid dreams, but difficult mornings and sleeping through alarms. This is a perceived benefit alongside an unwanted effect, without objective sleep testing. It does not establish an insomnia treatment.

04 / Reddit · Tolerability

Symptoms that prompted emergency care

One person describes good initial results alongside local irritation and other reactions. Several weeks into a later course, itching, hives, tingling lips and tongue and an unusual swallowing sensation prompt emergency care. We have no final diagnosis or product verification, and prior tolerance does not guarantee future tolerance.

SAFETY

Safety and possible
side effects.

These data come from the first 26 weeks of the historical Egrifta trials: 543 treated participants and 263 on placebo, with HIV/lipodystrophy.

FDA · Table 1 · percentage of people in each arm
Reported reactionTesamorelin
n = 543
Placebo
n = 263
Injection-site reactions · grouped term17%6%
Joint pain13%11%
Pain in extremities6%5%
Muscle pain6%2%
Peripheral oedema6%2%
Tingling5%2%
Reduced sensation4%2%
Rash4%2%

The table uses the grouped local-reaction term: 17% vs 6%. Narrative warning 5.6 reports 25% vs 14% for any local reaction; different aggregations should not be mixed. An event during treatment does not alone establish causation. Frequencies cannot be transferred to unregulated products. ↗

IGF-1 and glucose

At 26 weeks, 47% of treated participants had IGF-1 above +2 standard deviations and 36% above +3. These are marker elevations, not percentages with a new disease. HbA1c reached ≥6.5% in 5% on treatment and 1% on placebo. Glucose and IGF-1 assessment are part of medical supervision. ↗

Fluid retention and allergies

Retention can cause oedema, joint pain and carpal-tunnel symptoms. Hypersensitivity was reported in about 4% of treated participants. A new reaction should not be taken as a sign that the product “works”. Uncontrolled products may add identity, impurity and sterility problems that pharmaceutical trials do not quantify. ↗

Contraindications and special situations

The US label contraindicates use in active cancer, pregnancy, known hypersensitivity and certain hypothalamic–pituitary disruptions. Cancer history requires careful assessment; GH/IGF-1 stimulation does not establish that the peptide automatically causes cancer. Paediatric safety is unestablished, and molecule-specific breastfeeding data are absent. ↗

Other medicines and critical illness

Changing GH can affect metabolism of some medicines and conversion of cortisone to cortisol in people receiving glucocorticoid replacement. The label also urges caution in acute critical illness, informed by experience with pharmacological GH. Forum accounts do not establish a safety profile for peptide combinations. ↗

Swelling of the lips or tongue and difficulty breathing or swallowing require emergency medical help. Unusual thirst, frequent urination, marked oedema or painful hand numbness need medical assessment.

06 / FAQ

Your questions.
Clear answers.

The dossier, explained simply. Open a question for the answer and its source.

What is tesamorelin and what does it do?

Tesamorelin is a synthetic GHRH analogue, stimulating the pituitary to release GH. Its clearest clinical effect is less visceral fat in adults with HIV and lipodystrophy. It does not use the GLP-1 mechanism.

Source & context ↗

Does it help reduce abdominal fat?

Yes, in the studied HIV/lipodystrophy population. At 26 weeks the FDA analysis reports −18% vs +2% and −14% vs −2% in the two trials. This is CT-measured visceral fat, not all visible fat under the skin.

Source & context ↗

Why can fat decrease without much weight loss?

Visceral fat is only one part of body weight. In the pivotal trials, weight stayed nearly stable while trunk fat and lean mass changed. Water can contribute to measurements. The scale does not separate these components.

Source & context ↗

Are there trials in people without HIV?

Yes. A trial in 60 adults with abdominal obesity and reduced GH reported −16 vs +19 cm² of visceral fat at 12 months. This was a selected population, and the result does not confirm efficacy for every overweight adult.

Source & context ↗

Can it help with fatty liver?

A phase 2 trial in people with HIV and fatty liver found an adjusted difference of −4.1 percentage points versus placebo at 12 months. It does not establish reversal of existing fibrosis or efficacy in every form of fatty liver without HIV.

Source & context ↗

When were results evaluated?

The pivotal trials assessed the main outcome at 26 weeks. Other trials followed participants for 12 months. These measurement times do not offer a guaranteed schedule for visible changes in an individual.

Source & context ↗

What happens after stopping?

In trial extensions, switching to placebo was followed by regain of some visceral fat. Increases of +22% and +16% are relative to week 26, not treatment initiation. Effects were better maintained with continued treatment.

Source & context ↗

Is it the same as HGH?

No. Tesamorelin stimulates release of the body’s own GH; HGH supplies the hormone directly. They are different interventions with their own evidence and contraindications. Tesamorelin’s response depends on a functioning hypothalamic–pituitary axis.

Source & context ↗

Does it build muscle or improve gym performance?

Observed increases in lean mass do not automatically demonstrate new muscle or greater strength. This dossier contains no robust demonstration of a general sports-performance benefit. Online accounts often include other substances and changes to sleep or training.

Source & context ↗

Does it improve sleep or memory?

Some people describe better sleep, others difficult mornings; these are personal accounts. The 2025 cognition trial in 73 people with HIV found no significant main difference versus usual care. It does not confirm an insomnia or memory benefit.

Source & context ↗

What are the main side effects?

Trials report local reactions, joint pain, oedema, muscle pain and tingling. Risks also include hypersensitivity, elevated IGF-1 and altered blood glucose. Table percentages come from historical pharmaceutical trials, not unregulated products.

Source & context ↗Explore the Side effects section ↗

Can it increase diabetes risk?

Yes, the approval trials show a signal: HbA1c reached ≥6.5% in 5% on treatment and 1% on placebo at 26 weeks. A small 12-week trial in stable diabetes found no overall deterioration, but does not eliminate risk in other populations.

Source & context ↗

Does higher IGF-1 mean a better result?

Not necessarily. It is a marker of hormonal response, not a benefit score. Effects of persistent elevations are not fully known, and the US label requires monitoring. Elevated values are not themselves a cancer diagnosis.

Source & context ↗

When is it contraindicated?

The US label includes active cancer, pregnancy, known hypersensitivity and certain hypothalamic–pituitary disruptions. Cancer history, glucose, IGF-1 and other medicines require individual medical assessment.

Source & context ↗

Are EGRIFTA WR and SV interchangeable?

No. FDA states that the formulations are not substitutable: dose, presentation, preparation and storage differ. Vial quantity is not the same as the administered dose. Historical trials are not instructions for converting between products.

Source & context ↗

Is it authorised in Europe and available in Romania?

The European Egrifta application was withdrawn in 2012. US approval does not establish Romanian availability. Stock, import and reimbursement were not checked in this research; no purchasing recommendations are provided.
