THE ESSENTIALS

A familiar signal.
Built to last.

GLP-1 is a hormone made by the body. Semaglutide is a modified peptide that mimics some of its effects and lasts longer in the body. ↗

31 AMINO ACIDS

Chain schematic, without its three-dimensional shape. Highlights mark the two substitutions and the side-chain attachment site; they do not represent atoms.

Why does it stay active longer?

Its modifications protect it from rapid breakdown. A side chain helps it bind to albumin, a blood protein. Think of a molecule that circulates longer, rather than a brief pulse of a natural hormone. Molecular design ↗ 31 ↗

The same substance. Products with different purposes.

Status checked on 14 October 2026. EU authorisation does not confirm stock in Romanian pharmacies. Formulations are not interchangeable milligram for milligram.

WHAT YOU MAY NOTICE

Finding it easier to say:
“I have had enough.”

Less hunger does not mean the disappearance of every craving. And one hungry day cannot tell you whether treatment is working.

The signal: hunger may become easier to manage.

In a trial of 72 adults with obesity, after 20 weeks, the semaglutide group consumed 35% less energy at a test lunch than the placebo group. That was a laboratory meal, not a guaranteed daily calorie reduction. Friedrichsen · 2021 ↗

The meal: you may feel satisfied with less food.

Stomach emptying is only part of the story. The 20-week trial did not detect a delay with its measurement method. Another trial, in 20 women with obesity and polycystic ovary syndrome, found a delay after 12 weeks. The population, dose and measurement method differed. 2021 ↗ 2023 ↗

Over time: the feeling may change while an effect continues.

A 2026 trial of 120 adults found lower intake at test meals through 60 weeks. Some differences in reported hunger were no longer significant at weeks 40 and 60. “I feel hungry again” and “there is no effect left” are not the same statement. 2026 ↗

BEYOND THE SCALE

Weight is
only part of the story.

Heart and kidney outcomes were assessed in different trials, in people with defined conditions. We follow the results without extending them to everyone.

SELECT · HEART

A percentage becomes clearer
when you see the counts.

17,604 adults aged 45 or older, with cardiovascular disease, BMI ≥27 and no diabetes. Injectable semaglutide 2.4 mg/week or placebo; mean follow-up 39.8 months.

Outcome: cardiovascular death, non-fatal heart attack or non-fatal stroke.

Placebo8.0%
701 / 8,801 participants with a first event

About 6.5 in 100, compared with 8 in 100. The “20% lower” figure describes a relative reduction calculated over follow-up (HR 0.80). It does not mean 20 people saved in every 100. Bars show percentages.

SELECT ↗

FLOW · KIDNEYS

24% lower

Fewer major kidney disease events or cardiovascular deaths.

In 3,533 people with type 2 diabetes and chronic kidney disease, injectable semaglutide 1 mg/week reduced the combined risk being tracked by 24% versus placebo (HR 0.76) over a median 3.4 years. Researchers tracked kidney failure, a sustained loss of at least half of filtering capacity, or kidney/cardiovascular death. FLOW ↗

ESSENCE · MASH

WHAT COMES AFTER WEIGHT LOSS

The result deserves
a long-term plan too.

Maintaining weight and the ability to move well matter as much as the number on the scale.

AFTER WITHDRAWAL

Weight can return.

In the STEP 1 extension, with 327 participants, the previous semaglutide group regained about two-thirds of its weight loss within a year of withdrawal. Structured lifestyle support also ended.

Relative to starting weight, in the followed subset. This is not the full STEP 1 sample. STEP 1 extension ↗

BODY COMPOSITION

Weight loss is not exclusively fat.

The exploratory STEP 1 DXA analysis included 140 people. In the semaglutide group, fat mass fell 19.3% and lean mass 9.7% at 68 weeks.

The percentages use different starting amounts: initial fat mass and initial lean mass. Do not add them. “Lean mass” is not just muscle; this measurement alone does not show how much strength someone lost. DXA ↗

PERSONAL ACCOUNTS

The same molecule.
Different experiences.

Three accounts read on Drugs.com, paraphrased. An editorial selection, not a survey or evidence of an effect’s frequency or cause.

01 / Drugs.com

Smaller portions, earlier fullness.

After weight gain during the pandemic and menopause, one person describes smaller meals and earlier fullness. They report feeling less hungry without feeling unwell.

02 / Drugs.com

Pleased with the result, despite digestive discomfort.

Eight weeks in, one person was pleased with the weight loss. Injection-day tiredness, gas and occasional diarrhoea remained; they felt the result was worth that discomfort.

03 / Drugs.com

Lost weight but could not tolerate treatment.

One person lost weight over six weeks, but nausea and reflux made eating difficult. After stopping, they report feeling better within days and quickly regaining weight.

SIDE EFFECTS

What is worth knowing.
Without guessing.

The most frequent effects are digestive. They can be mild, but sometimes become difficult to tolerate. More discomfort does not mean a better result.

Discuss beforehand

Tell your clinician about pancreatitis, gallbladder problems, severely delayed stomach emptying, retinopathy and diabetes medicines. Inform the medical team before anaesthesia. ↗

In the EU, semaglutide is not used in pregnancy; discontinuation should be planned at least two months before a planned pregnancy. ↗

Rare but relevant risks

EMA classifies NAION, an optic nerve condition, as a very rare reaction. Sudden vision loss requires immediate assessment. EMA ↗

The US label contraindicates Wegovy with a personal/family history of medullary thyroid cancer or MEN2. The warning stems from rodent tumours; human relevance is not established. ↗

When to seek help

Severe persistent abdominal pain or vomiting that prevents keeping fluids down needs prompt assessment. Difficulty breathing or swelling of the tongue/throat is an emergency: call 112 in Romania/the EU or your local emergency number. ↗

06 / FAQ

Your questions.
Clear answers.

The dossier, explained simply. Open a question for the answer and its source.

What is semaglutide and what does it help with?

Semaglutide mimics the GLP-1 signal involved in hunger, fullness and blood sugar. It is used in medicines for type 2 diabetes and weight management, depending on the product and eligibility. Benefits and side effects vary between people.

Source & context ↗

Is semaglutide the same thing as Ozempic?

Semaglutide is the active substance; Ozempic is one medicine containing it, authorised for type 2 diabetes. The product’s indication and formulation matter.

Source & context ↗

Who is it authorised for in weight management?

In the EU, Wegovy is intended for adults with BMI ≥30, or ≥27 with a weight-related health condition, alongside diet and physical activity. The injectable form also has an indication for certain adolescents aged 12 or older. Eligibility requires medical assessment.

Source & context ↗

How quickly do you lose weight, and how much?

There is no certain figure for each person. STEP 1 measured a mean reduction of 14.9% at 68 weeks, versus 2.4% with placebo. An average cannot tell you what will happen in the first month or to you.

Source & context ↗

If I still feel hungry, does that mean it is not working?

That feeling alone cannot establish this. In the 60-week trial, some differences in reported hunger diminished while test-meal intake remained lower. Progress needs to be assessed over time.

Source & context ↗

What happens if I stop treatment?

Weight can return. Withdrawal data support discussing a maintenance plan with your clinician, rather than assuming the result will automatically persist. See the STEP 1 extension above.

Source & context ↗

Can muscle be lost too?

Lean mass can decrease alongside fat. DXA does not measure only muscle and does not replace strength assessment. See the body-composition section.

Source & context ↗

Do I need to feel nauseated for it to work?

Nausea is an adverse reaction, not a target. Persistent or hard-to-tolerate symptoms deserve discussion with your clinician. The dedicated section shows reported frequencies and signs that need attention.

Source & context ↗Explore the Side effects section ↗

Is semaglutide also available as a tablet?

Yes. In the EU, Rybelsus is available for type 2 diabetes and Wegovy tablets for weight management in eligible adults. They are not equivalent milligram for milligram to injections; switching formulations requires a clinician.
